This front hub failed when the radial spokes tore out the flange. The hub is from a Rolf Sestriere wheel supplied with a new 2000 Trek 5900 USPS Superlight bicycle. The wheel was in storage, not even in use when failure occurred.

Radial spoking is safer with a moderate number of spokes to place less tension on a beefy hub flange. This wheel had only 20 spokes, but very high spoke tension and weak flanges left this hub prone to failure.
One odd feature of Rolf hubs is the smooth surface to engage the dropout. A serrated, steel surface is usual, to secure the wheel better.
