Most bicyclist fatalities occur in car-bicycle collisions, while most brain injuries occur in simple falls, which are are hundreds of times more common than fatal car-bike collisions. There is nothing about Portland to suggest that falls are less likely than in any other American city. There are some things such as streetcar tracks which suggest that falls might be more likely. At the right in the photo, though, is a rather practical cargo bicycle.
